Respectful Care – To receive considerate, respectful treatment in a safe environment, free from discrimination based on race, ethnicity, national origin, religion, sex, age, health status (including mental or physical disability, genetic information, medical history, or domestic violence), or sexual orientation.
Continuity & Scheduling – To expect continuity of care, know the name of your provider, and be advised in advance of appointment times.
Information & Participation – To be informed of your health status and actively participate in developing and implementing your treatment plan with your physician and other providers.
Informed Consent – To accept or refuse any test or treatment and receive information on the consequences of refusal.
Procedure Explanation – To have any proposed procedure explained (benefits, risks, alternatives, and costs).
Second Opinions & Referrals – To receive assistance obtaining consultations or second opinions with other Non-Northeast Rehab providers, and to have your care transferred outside our practice if desired.
Ownership Disclosure – To obtain information about any ownership interests your physician has in referred services or facilities, and to choose an alternative facility if you wish.
Privacy & Confidentiality – To personal privacy during treatment and to expect that your medical records and communications are kept confidential in accordance with state and federal laws.
Access to Records – To access your medical records (copying fees as permitted by law).
Billing Transparency – To examine your bill and receive an explanation of charges for services rendered.
Grievance & Complaint – To register complaints and seek resolutions regarding your care without fear of reprisal.

Honest Communication – To provide accurate and complete information about your health status, complaints, medications, and medical history.
Treatment Compliance – To follow the treatment plan and staff instructions; if you refuse or do not follow instructions, to accept the consequences.
Questions & Concerns – To communicate with your provider about questions or concerns regarding your treatment and ensure you understand your care plan.
Reporting Changes – To report changes in your condition or symptoms, including pain, to your provider.
Appointments – To keep appointments or notify us in advance if you cannot attend.
Consideration of Others – To be considerate of other patients, staff, and property.
Practice Policies – To follow practice rules and regulations, including but not limited to, our No Tobacco, No Weapons, Zero Tolerance and Animal Assistance policies.
Financial Obligation – To provide accurate insurance information and to pay your bills or make payment arrangements with our billing department.
